About Us

• The Fountainss Alzheimers Care, Dementia
  Memory Loss Care Services are focused firmly on
  the individual and on providing a safe, familiar,
  yet stimulating environment. The Fountains
  neighborhood is specially designed to provide
  comfort and security in a separate environment
  for those with Alzheimers disease, dementia and
  other forms of memory loss. Our team is trained
  to value each senior and to recognize their
  different preferences and needs.

• The Fountains Reminiscence Program
• At The Fountains, we have a specially
  designed Reminiscence Neighborhood for seniors
  with Alzheimers, dementia or other forms of
  memory loss. We begin with understanding the
  stories and details of a seniors life so we can
  help all experience pleasant days.
  Our professional team is dedicated to maintaining
  the dignity and individuality of every resident.
  They design personalized programs with
  multi-sensory experiences, daily exercise and
  outings, group activities and regular contact
  with children and pets, depending on what
  benefits each individual senior.

• Individualized Service Plan (ISP), Services
  Amenities
• At The Fountains, we believe that every
  individual has their own needs and preferences,
  and so we meet with each resident and family
  before they move in and at regular intervals
  afterwards to help determine appropriate care
  and service. A team of our community
  professionals comprised of the executive
  director, Reminiscence coordinator, health care
  coordinator, and designated care manager work
  together to create an individualized plan that is
  monitored on a daily basis and is updated based
  on the individuals changing needs.
