  • Infertility is a medical condition which can be
    treated and both female and male suffer from
    Infertility worldwide. Female infertility is not
    being able to conceive after having regular
    unprotected sex for an year or not being able to
    carry the pregnancy till term and having repeated
    miscarriages. Female factor accounts for
    approximately one third of all infertility cases.

Causes
Fertility declines with age as ovarian reserve
starts declining in late 20s or after 30 years
and the quality of the remaining eggs is lower
than younger females.
In order to become pregnant egg must be released
from ovaries i.e ovulation, egg must travel
through fallopian tube to uterus, sperm must join
with egg in fallopian tube and fertilized egg
must get attached to uterine wall.

Causative factors of female infertility can be
many but the most common of them is the ovulating
factor. A woman who is not ovulating due to age,
early onset of menopause or may be releasing
inferior quality of eggs will suffer from
infertility. PCOS( Polycystic ovary syndrome )in
which multiple follicles are formed in the
ovaries but they are not able to release eggs
results in infertility. Thyroid problems like an
overactive or underactive thyroid gland can lead
to problems in ovulation due to hormonal
imbalance. Lifestyle issues like smoking,
obesity, drinking, stress also affects in
ovulation.
Ovary releases the egg into the fallopian tube.
It is where the egg and the sperm meet and
travels to the uterus for implantation. Sometimes
due to blockage in the fallopian tube the egg
released by the ovary is unable to make its way
through it or preventing the sperm from
travelling towards the egg for fertilisation. The
fallopian tubes might get blocked due to reasons
like PID (pelvic inflammatory disease). Sexually
transmitted diseases (STD) like Gonorrhea or
Chlamydia might cause an infection in the uterus
and result in PID. When a fertilised egg gets
implanted in the fallopian tube instead of uterus
known as Ectopic pregnancy can also affect the
passage of the fallopian tubes. Any previous
surgery performed might cause changes in the tube
and the uterus. Tuberculosis in the pelvic area
is also a known factor in the third world
countries like ours. Scarring happens due to
Endometriosis which made tissues grow outside
uterus and create tubal blockage. It might also
damage the lining of the uterus preventing the
fertilised egg to get implanted.

In chronic conditions like Chemotherapy drugs or
radiation given for cancer treatment close to the
reproductive area also sometimes renders a woman
infertile.
In some women, Uterine factors or cervical
factors also leads to infertility .Congenital
defect in the cervix might prevent sperm to
travel to the fallopian tube for fertilization or
uterine malformations can lead to female
infertility. Polyps, fibroids in uterus results
in distortion of uterine cavity making it
difficult for implantation. Scarring or adhesions
from previous surgery or infection also can cause
infertility. Cervical mucus defect is also one of
the causative factors.
In some women the tests come clean but they fail
to conceive. This is known as unexplained
infertility.

How female infertility is diagnosed? With the
development of ART, treatment of infertility has
improved a lot. Various tests are conducted to
diagnose the problem. Ovulation A blood test
is performed to understand the level of
Luteinizing hormone (LH) to know the surge of
this hormone which impacts ovulation. Another
test involving Progesterone (a hormone produced
after ovulation) level in the blood is also
tested to confirm ovulation.
Hysterosalpingography A dye is injected into
the uterus and x-ray is taken to detect
abnormalities. Whether the dye is spilling into
the fallopian tubes is also checked. Sometimes
this procedure helps to open the fallopian tubes
and improves fertility.

Ovarian reserve testing A series of blood and
image testing is done to assess the quality and
quantity of eggs. Tests like AMH(anti mullerian
hormone) and AFC(antral follicle count) are the
best predictors of ovarian reserve. Production of
eggs becomes less with age. In women older than
35 years the egg supply gets depleted and such
women generally undergo these tests.
Imaging tests Sometimes an ultrasonography is
performed around pelvic area to rule out any
fallopian tube or uterine disease. Other hormone
testing Hormones produced by thyroid and
pituitary glands control reproductive processes.
Checking levels of these ovulatory hormones helps
in indicating infertility. To diagnose
infertility problems and depending on the
situation some more tests are also performed.

Hysteroscopy A viewing scope is used to look
for any uterine or fallopian tube disease or
disorder
Laparoscopy A minimally invasive procedure is
performed through which a viewing device is
inserted to identify scarring, endometriosis or
blockages. It can also detect problems of the
ovaries and uterus.
