ANATOMY AND HISTOLOGY
The Thyroid gland is a lobular endocrine gland
that is inferior to the neck, anterior and
larynx. Grossly, the gland appears brownish-red
and the left and right lobes are joined by an
isthmus.

The main purpose of this organ is to produce,
store and secrete the iodine-based hormones
triiodothyronine (T3) and Thyroxin (T4). These
hormones have various effects on fat, protein,
and carbohydrate metabolism, as well as
specifically on the central nervous system and
general development.
5
Regulation of Thyroid Axis
TSH
Thyrotrope cells of ant. Pituitary 31 kDa
hormone a and ß subunits a subunit similar to
LH, FSH and HCG Stimulated by TRH TSH, TRH
suppressed by Thyroxin Pulsatile secretion
6
Iodine primarily derived from diet RDA of
iodine Adults 150 µg Children 90 120
µg Pregnancy 200 µg Urinary iodine is gt10 g
/dL in iodine- sufficient Plasma iodide partly
replenished from thyroid

9
Atrophic hypothyroidism
Hashimotos disease (autoimmune thyroiditis
type 2B) Post ablative-follows surgery,I131
radiation
Congenital
Thyroid agenesis or dysplasia (80-85 of
neonatal hypothyroidism) TSH receptor defects
Thyroidal Gs protein abnormalities Idiopathic
TSH unresponsiveness

Transient (post thyroiditis)
hypothyroidism Following subacute.painless, or
post partum thyroiditis Consumptive
hypothyroidism
D3 over expression in hemangiomas
