Obstetric Fistula
An obstetric fistula is a small channel between
the vagina and rectum or bladder that is caused
by prolonged obstructed labour when the unborn
baby cannot pass through the pelvis.

Types of Obstetric Fistula
  • Vesico-uterine Fistula between the urinary
    bladder and uterus.
  • Vesico-vaginal Fistula between urinary bladder
    and vagina.
  • Urethro-vaginal Fistula between the urethra
    (tube that carries the urine out of the bladder
    for excretion) and vagina.
  • Rectovaginal Fistula between the rectum and
    vagina.

Causes
  • Prolonged obstructed labour.
  • Low-quality c-section.
  • Lack of access to c-section.
  • Cervical cancer therapy.
  • Injury during vaginal, anus or rectum surgery.
  • Inflammatory bowel disease like ulcerative
    colitis, Crohns disease

Symptoms
  • Fecal Incontinence or urinary incontinence, which
    may be continual or just occur at night.
  • Discharge from the vagina which may be
    foul-smelling.
  • Vaginal or urinary tract infections.
  • Irritation, pain or swelling around the vaginal
    area.
  • Pain during intercourse.
  • Nausea/Vomiting
  • Abdominal or pelvic pain
  • Irritation or soreness inside the vaginal area

Obstetric Fistula Treatment
  • The surgical closure of an Obstetric Fistula is
    the most effective way to treat this condition.
  • About 80 to 90 of women with Vesico-vaginal
    Fistula can be cured by a simple vaginal surgery.
    The more complicated fistulas may require
    multiple attempts before continence is achieved.
  • Recto-vaginal Fistulas is repaired in layers.
    Primary reconstruction is done. Sphincter repair
    is done. This is followed by vaginal wall repair.
    Finally, the rectal mucosa is repaired.
