Speaker Disclosure Explanation

• As an accredited provider of CME, the University
  of Oklahoma College of Medicine Office of CME
  must insure balance, independence, objectivity,
  and scientific rigor in all of its individually
  sponsored or jointly sponsored educational
  activities.
• Speakers, moderators, or panelists in an
  accredited CME activity, are required to disclose
  to participants
• Receipt of funds or other things of value from
  proprietary entities,
• Discussions of off-label or investigational
  products or devices not labeled for the proposed
  use,
• Existence of a significant financial interest(s)
  or relationships(s) with manufacturers or
  commercial products and
• 4. Relationship(s) with the commercial supporters
  of the CME activity, if applicable.
