CLIENTS RIGHTS
  • UCP believes that each person that receives
    services through the Agency has the right to
    use/receive proper professional services, to
    enjoy privacy and confidentiality in the use of
    information about his/herself and to be free from
    embarrassment, discomfort and harassment. People
    have the right to
  • Access the established procedures for handling
    their complaints
  • Be informed of services available.
  • Communication concerning program.
  • A safe and humane service environment.
  • Be protected from abuse, neglect or mistreatment.
  • Privacy which promotes their quality of life.
  • Receive appropriate quality service.
  • Participate in the planning of their service.
  • Prompt and appropriate medical care and emergency
    medical treatment.

SECLUSION / USE OF RESTRAINT (CHILDREN)
  • We practice nonaversive, positive behavior
    management techniques.
  • Praising appropriate behavior
  • Limits and rules are expressed
  • Simple verbal interpretations
  • Some bad attention-seeking behaviors are ignored

  • Time Out used when behavior is aggressive or
    persistent
  • Time-Out Log Sheet
  • Use of seat belts
  • Seat Belt Use Log Sheet
  • Under no circumstances will the staff of UCP use
    physical punishment or any other procedure which
    might humiliate, hurt or frighten a child.

SECLUSION / USE OF RESTRAINT (ADULTS)
  • It is the policy of UCP that behavior management
    programs be designed according to the individual
    needs of the client and that special attention be
    directed toward the protection of the rights of
    the individual with whom such techniques are
    employed.

SECLUSION / USE OF RESTRAINT (ADULTS)
  • Use of corporal punishments is prohibited
  • Physical restraints or isolation are prohibited
    unless the behavior is detrimental to the person,
    the staff or the general public.
  • If restraints are needed, it will be noted in the
    clients file.

ELOPEMENT / WANDERING
  • Elopement unassisted, unsupervised departure
    from the facility requiring the implementation of
    a missing person protocol.
  • Wandering cognitive impairment resulting in a
    client moving about inside or outside a facility
    without an appreciation for personal safety.

ELOPEMENT / WANDERING
  • Observable Elopement
  • Staff will attempt to deter an elopement by using
    the following procedures
  • As the client moves away from the site, alert
    other staff members you will be following the
    client and keep the client in your sight at all
    times.
  • Return the client to the UCP site as quickly as
    possible

  • Out-of Sight Elopement
  • Use crisis protocol
  • Begin to look for the client on surrounding
    property. Notify second staff before you leave
    the premises.
  • If you are unable to locate the client within 5
    minutes, contact the Program Director.
  • Call the police.
  • Continue to look for the client until the police
    arrive. Contact the clients emergency contact.
  • If the client is found after the police are
    called, but before they arrive, notify them
    immediately.

  • Reporting Adult
  • If not located within 30 minutes, contact
    parent/guardian and contact RCS
  • Within 24 hours of the incident, fill out an
    Incident Report Form and turn into Safety
    Chairperson.
  • Within 72 hours of the incident, fill out a RCS
    written report.
  • Within 15 working days of the incident, fill out
    a RCS Investigative Report.

  • Reporting Youth
  • If not located within 30 minutes, contact
    parent/guardian immediately.
  • Within 24 hours of the incident, fill out an
    Incident Report Form and turn into Safety
    Chairperson.
  • Safety Committee
  • Responsible for building inspections to determine
    if locks on doors and windows are safe from
    elopement
  • Review the missing client protocol annually and
    revise as necessary

MEDICATION ERRORS (ADULTS)
  • Level 1
  • Incidences that require no treatment or
    intervention other than observation
  • Level 2
  • Incidences where the client experiences
    short-term, reversible adverse consequences and
    treatments are needed
  • Level 3
  • Incidences in which the client experiences
    life-threatening and/or permanent adverse
    consequences

MEDICATION ERRORS (ADULTS)
  • Level 1 Reporting
  • File a written report with RCS within 72 hours
  • Level 2 Reporting
  • Verbal report within 24 hours of the incident
  • File a written report with RCS within 72 hours
  • Level 3 Reporting
  • Verbal report within 24 hours of the incident
  • File a written report with RCS within 72 hours
  • Will have extensive follow-up with RCS

  • Medication Authorizations
  • Physical symptoms of an overdose
  • Abnormal breathing
  • Slurred speech
  • Lack of coordination
  • Slow or rapid pulse
  • Low or elevated body temperature
  • Reddish face
  • Heavy sweating
  • Drowsiness
