G-P-R-A, GPRA, GPRA, GPRA!
  • Translations we needed them, we got some of
    them, we need to show that were using the ones
    we have if we want to get other languages
    translated.
  • SAMHSA Ultimately our project will be judged on
    our GPRA ability.

GPRA Procedures Reviewed
  • The GPRA is to be completed within 1-4 days after
    entering treatment.
  • Once the GPRA is completed it needs to be entered
    within 7-10 business days.
  • a. the person who did the GPRA should be the
    one to enter it since they are most familiar
    with the answers.
  • b. all people entering on the GPRA system need
    a user name and password that is unique to
    the individual. To do this e- mail
    csat-gpra_at_westat.com and provide them with the
    following information
  • Name
  • Phone
  • Email Address (must be unique to the
    individual)
  • Grant TI-17817
  • c. CC me on the above e-mail so that I know you
    have done this.
  •

  • Each clinic site will be in charge of assigning a
    unique identifier and they should be in the
    following format
  • Hamilton Madison House
  • HMH001_ _ _ _ (the last 4 digits of the
    clients SS , if they dont have one use 0000)
  • Queens Child Guidance
  • QCG003_ _ _ _ (the last 4 digits of the clients
    SS , if they dont have one use 0000)
  • QCG002 has already been used!

GPRA Details
  • Confidentiality
  • The GPRA tool is not to be kept with the
    patients clinic records. It should be stored
    separately in a locked file cabinet, separate
    from any paper work that has identifying or
    linking information.
  • Follow up
  • Be sure to update your Locator form for the
    client every time they present for a treatment
    appointment, this way when it comes time to
    complete the follow up GPRAs your information
    will be up to date and more accurate.
  • The 6 month GPRA can be completed as early as the
    5th month or as late as the 8th month.

  • If the 5th month is approaching and the client is
    about to be discharged as well, the 6 month
    follow up survey can be completed once, and
    entered twice, first as the 6 month and second as
    the discharge.
  • If you are ever unclear about a GPRA procedure
    ask someone who has attended the GPRA training.
    Read through the materials that they have
    received, utilize the help desk, or you can
    always ask me.
  • The minimum follow up rate is 80

    procedures are in place
  • Everyone GPRAd should sign a consent and release
    of information form.
