Next you need to complete details of the incident
(what happened)
  • For completing the information on
  • Incident Type
  • Incident Severity
  • Potential Risk Rating
  • You should use the Incident Notification,
    Investigation, Reporting and Follow-up Guidelines
    (HSE/97/01).

Lets run through an example
A helper was fixing a safety plaque on the wall
of the DSVs office. He had drilled the hole in
the wall, put in the raw plug and was fixing the
screw. He didnt have the right sized screwdriver
with him, so he was using a knife he had found
nearby. In the process of fixing the screw the
knife slipped and sliced through his forefinger
on his left hand, which he was using to steady
the screw. He was taken to the medic who cleaned
and dressed the wound with butterfly stitches and
advised him that he could return to work but
should take it easy for a few days. The helper
was then sent to the PDO clinic to check that the
correct diagnosis had been given.

you need to use the HSE Risk Matrix...

To work out the actual severity you need to
identify the
  • Severity Rating

and then the
  • Consequence

In this example the helper was injured but can
return to work to do light duties i.e. A
Restricted Work Case
  • the severity is 2
  • and the consequence is a People Injury or P

therefore the Actual Severity Rating is 2P

To work out the Potential Severity you need to
identify the
  • Probability of it
  • happening again
  • the severity
  • the consequence

In this example
Probability - this type of accident is likely
to happen gt5 times per year in PDO - D.
Severity - the potential severity is an LTI -
3.
Consequence - the potential consequence is
People Injury - P.
therefore the Potential Severity is D3P
The incident is classified as a MEDIUM POTENTIAL
INCIDENT

When completing the actual and potential severity
rating it is important to note
  • A Near Miss is something that has not resulted
    in an accident
  • however it has the potential to be an accident.
  • the Actual Severity is 0,
  • however the Potential Severity must be gt0

incident accurately and concisely.

before starting a job. To discuss at next Safety
meeting - requirement for hazard identification.

Distribution Lists
Once the incident form is complete it is
important to distribute the form to the relevant
people based upon the potential severity of the
incident.
  • In addition it is important to make sure that the
    correct people are copied for
  • Injurious incidents
  • Road Transport
  • Environmental
  • Radiation
  • Security and Theft
