  7.4 or less
• COPD
• To reduce inappropriate respiratory hospital
  admissions by 40

• Attendance of all 3 learning workshops
• Submit data monthly
• PDSAs
• Full project management support

• Invite patients for smoking cessation advice

What Next?

• Ensure patients under the hospital are identified
  and invited for review
• Group patient education sessions
• Set up clinics for patients with I.G.T.
• Spread to more practices
