which cause a majority of fatalities and injuries

Minimum 10 foot clearance
3Trenching/Excavation
Any trench four feet deep or more must be
protected.
4Guardrails
Openings with a drop of four feet or more must be
guarded
5Standard Guardrail Specification
- Top rail 3945 inches with mid-rail
- 200 pound load requirement
6Scaffolding Fully Planked and With Guardrails
7Stairways
Four or more risers must have handrails
8Fall Protection
Elevations ten feet or greater must be protected
9Fall Protection Work plan
10Open Belts Pulleys, Chains and Sprockets
11Power Saws Must Be Guarded
12Chainsaws
Leg, face , hearing head protection required
13Angle Grinders - 180 Guard Required
14Ladders
- Extend 36 inches above landing
- Secure to prevent displacement
15Articulating Boom Lift
Full body harness required at all times
16Floor Holes/Openings
Covered and secured with no tripping hazards.
17Electrical Hazards
- All temporary power must be GFCI-protected
- All electrical circuits and connections must be
approved
18Minimum Clothing
At least a short-sleeve shirt, long pants and
substantial footwear required
19Hard Hats
- Readily accessible
- Must be worn whenever overhead hazards exist
20Compressed Gas Cylinders
Must be stored properly
21Vehicular Traffic Hazards
Employees must wear high-vis apparel
22Housekeeping
During construction, continuous cleanup is
required to eliminate tripping hazards
23First Aid Kits Training
First-aid supplies must be readily available
