    level preventative interventions that focus on

The objectives of the pilot
  • Create additional entry points into systems
  • Target those older people already in health,
    housing or social care systems but in addition
    may have needs that can benefit from this
    approach
  • Target those older people not within health,
    housing or social care systems and have low level
    needs

Methodology
  • Paper based tool distributed to older people
  • Electronic tool available via Council website
    which links into a resource directory
  • Assisted / mediated assessment for those that
    require a little bit more help to navigate options

    warmth scheme, home safety
  • Equipment - Minor aids e.g.
  • Advice and information about suitable equipment
    and reputable suppliers
