• Folic acid antagonist that inhibits DNA, RNA, and
  protein synthesis.
• Non specific cytotoxin (highly toxic to normal
  cells) requiring a careful individualized
  therapeutic regime
• High-dose therapy followed by leucovorin rescue
  36-42 hr after dose (counteracts the methotrexate
  effects)
• Leucovin rescue dosage is based on MTX serum
  concentrations.
• Serum concentrations are monitored during
  high-dose therapy to determine time and dosage of
  leucovin intervention.
• Serum MTX peaks at about 1-1.5 h
