Our Clinic

• Federally funded community health center serving
  the medically indigent residents of the Rockaway
  Peninsula, Queens, New York
• Community health center services include primary
  care, pediatric resource center, family support,
  mental health and HIV early intervention
  treatment

Challenges

• 1. Limited clinical information systems do not
  have the ability to identify, collect and analyze
  performance indicator data
• 2. Integrating HIV program improvements into
  overall clinic systems ( remember, we have an
  integrated model of care)

  most beneficial

• Data collection collect data on all core
  medical indicators
• Identify members of QI Team, including a champion
  who will facilitate agency buy in to QI program
• Set a date and time for 1st QI meeting

Lessons Learned In Implementing A Quality
Project

• Be persistent in spreading the word.
• Include staff from other departments and areas in
  the project team. They can carry the message
  back.
• Use quarterly all-staff meetings to introduce and
  train staff on changes.
• Overall organization policy changed to include
  new staff orientation because of frequent staff
  changes
• Set realistic goals to promote success
