• A 45 year old woman with obesity (BMI 34 kg/m2
  waist 90 cm) since childhood asks about
  eligibility for gastric bypass surgery
• Medical problems hypertension, dyslipidemia
• SH Failed numerous diets. Exercise limited by
  degenerative arthritis of knees
• Lab Data fasting glucose 120 mg/dl, cholesterol
  220 mg/dl, triglycerides 160 mg/dl, AST 60 U/L,
  ALT 40 U/L

• What are reasonable and salutary goals for weight
  loss?
• Is any diet or counseling program superior?
• What weight loss can be maintained long term?
• Are any drugs safe and effective long term?
• If a liver biopsy is done and shows NASH,
  should weight loss program be modified?
• Should bypass surgery be considered in light of
  comorbidities and prior failures with dietary
  therapy?
