- Evidence supplied will be recorded on the
checklist along with any notes - The Monitoring and Technical Assistance Checklist
must be signed by the Title IIA Coordinator and
the Title II A Consultant and dated - A copy of the completed checklist will be
supplied to the system Title IIA coordinator and
the superintendent.
