Background to Proposal
  • Proposal developed to pilot MoM in support of
    diabetes care pathway
  • Agreed CPEC (11th Mar 08) DCPG (20th Mar 08).
  • Submitted 25th Mar 08 - Accepted 11th Apr 08.
  • A service project with ICT support.
  • Overall aims within proposal
  • Assess how MoM can support care pathway
    development and use.
  • Test how localised MoM can be.
  • Develop a framework that other care pathways can
    use without having to learn lessons all over
    again.

  • Covers 28 medical specialities 390 pathways.

    guidance.
  • Can be localised with additional detail by
    certain users.

Medicine and viewable to all users

Proposal Scope
  • Covers Diabetes Care Pathway.
  • Create local framework for other pathways.
  • Establish user roles access types
  • Edit / Review / Approve / View etc. (more later)
  • Distribution of necessary guidance / support.
  • Agree training regimen.
  • Develop Diabetes pathways using MoM - test
  • whether incremental development possible / easy.
  • Localisation methods incl. local policies
    agreements.
  • Develop KPIs and baseline of information.

  • Integration with knowledge management
  • Ongoing educational aspect.
  • Communicating (new) availability to existing /
    new staff.
  • Ensure service ownership of Map of Medicine
  • Not ICT tool just ICT enabled.
  • Embed into ongoing service management debate.
  • Enable showcasing of Map of Medicine
  • Sell what lessons we have learnt expertise
    gained.
  • Production of lessons Learnt
  • To assist other internal / external colleagues.

Proposal Scope
  • Proposal for mainstreaming into service
  • Framework of policies procedures (incl. access
    security) for other pathways to be developed
    without need for new projects.
  • Ensure ongoing communication in place
  • New local practice via MoM advertised.
  • New national MoM developments known.
  • Method for new and refresher training.
  • Benefits realisation report produced
  • Report against immediate products produced.
  • Develop publish lessons learnt of short term
    findings.
  • Place date in diary for future benefits
    assessment.

Proposal Scope (Some Exclusions)
  • Just Diabetes at this stage.
  • Covers just PCT Acute Trusts?
  • Theoretically NOT Ambulance, BUT agreement gained
    to include Ambulance specifically Emergency
    Care Practitioners (ECPs).
  • Does not link to clinical systems (yet)
  • e.g. Diabetic Retinopathy.
  • Interfacing / invoking is another, later project.

Project Brief to be created
  • Designed to get everyone up to speed with same
    information (the what of the project).
  • Approach developed and refined in tandem.
  • Extension of Proposal to include
  • Organisational structures.
  • More detail on products to be produced.
  • Tentative schedule please remember this is a
    pilot
  • Each stage new piece of work requiring its own
    assessment of what is required, how achieved
    likely time to deliver.
  • We are all under resource pressure, so will be
    looking to see how MoM can be deployed alongside
    existing pathway work.
  • Consider initial communications risks.

  • Allows for decisions made in this meeting to be
    incorporated into Brief.
  • Requires sign off by Board i.e. YOU
  • When Brief agreed, it is further refined into a
    Project Initiation Document (PID) signed off
    when conditions are in place to start project
  • Control docs progress reports, issue / risk
    logs, test scripts, sign off conditions, lessons
    learnt etc.
  • Policy docs comms strategy / management of
    risk, business continuity, transitional
    arrangements etc.
  • More detailed stage planning
