2What is the Pilot?
- Nationwide Pilot Project funded by Dept
- Of Health and Dept of Education Skill
- To change transition reviews and planning,
- making sure the young person is at the
- centre of the process.
3How Southampton took part.
- A leadership group was formed to plan how we
- wanted to take things forward in Southampton.
- Attended regional presentations and made a
- Commitment for Pete Claire to take part in
- The training and facilitate person centred
- Transition review meetings for two young
- people.
4What this involved
- 9 days training, planning reflection with
regional group. - Liaison with Ridgeway House School and Rosewood
School. - Visited two young people and their families to
explain the pilot. - Visited the two young people at their school
- Supported the school staff to pre-plan for the
meeting. - Gathered feedback from people who attended.
5What Happens in a Person Centred Transition Review
- The Young person is always there at the beginning
of the meeting and stays for as long as they
want. - The room is set up in an informal way and
refreshments are available, background music is
played. - Flip charts are used to write down and record all
information during the meeting. - The meetings were facilitated by Pete/Claire, not
chaired by the Head Teacher. - Everybody is encouraged to contribute equally.

- Things that people who came thought were good.
- Fantastic, child focussed.
- Time consuming but still great
- More person centred across all aspects of her
life. - Young person was very involved, from planning it
down - to supervising the actual proceedings!

- Everyones contribution was valued equally.
- Focussed on her whole life and not just her
school life. - Much more interactive, more relaxed and all

- My name is David and I was chosen by my school in
Southampton, to take part in the - national pilot for person centred transition
reviews in schools. - Claire and Pete whose job it is to make sure that
I am fully involved in my review - meeting, came to see me and my Mum and sister at
my home in the summer of 2005. - Pete and Claire also came to see me at School
with my teacher and friends.

- They talked with me about what I wanted to happen
in my - meeting and I decided that I would like to start
off by - blowing my trumpet which I was learning to play
in school, - and play a video called The Rock Challenge
which - showed me taking part in a presentation with my
school. - I also had some of my art work on display and
used - pictures of activities I enjoy for me to stick on
the flip charts - on the wall.
- One of the actions from my year 9 review was for
Mum to - take me to see my doctor after I told everyone
that my feet - hurt if I walked too far.

- On 20th April Pete and Claire invited me and my
mum to the South East - Regional Celebration Day at The Sustainability
Centre. - I had to come in a wheelchair because after
seeing my doctor I was - referred to a consultant who advised surgery to
correct a problem with my - foot.

At lunchtime we watched the dogs rounding up the
sheep. One of the dogs accidentally jumped onto
my lap which gave me a shock. We then watched a
lamb being fed.

In October 2006 I had my Year 10 review which was
again facilitated by Pete Claire. We started
with what everyone liked and admired about me
which had written up at my last meeting. It was
different this time as we were thinking more
about what would be available when I leave
School. We listed all the things that were
important to me for the future under the
following headings Self Determination Direction M
oney Home Support Community Life

- I invited two of my friends into the meeting at
this point to help me decide which were the most
important subjects to talk about, and these were
highlighted by stickers which we called dot
voting. - At the end of the meeting Pete and Claire drew up
an action plan and discussed the points that had
the most dots around it first. Each action was
given a date and the name of the person who was
responsible for seeing that it was carried out. - I would like to go to college when I leave school
and have also thought about becoming a nurse

- In December I was invited to
- give a talk at a person
- centred planning conference
- at The Novotel at Southampton
- in front of 120 people.
- Pete asked me a few questions
- about what was different at my
- review meetings, and then I
- interviewed my head teacher
- about what she liked about the
- meeting

- I think it would be good if more young people
in Southampton could be offered this style of
meeting. - Pete and Claire are meeting with the schools and
the city council to see what can be done to make
this happen
21Whats Happening Now
In July we recruited a person centred worker
funded by Childrens Services, Phil Orchard.
(Teacher/Pastor) Based at Choices Advocacy and
line managed by Pete. Concentrating on
developing person centred reviews in schools and
facilitating person centred plans for young
people in transition
22Whats Happening Now
- Meetings with Childrens Services
- Chaired by Jamie Schofield
- (Service Manager-Children with Disabilities)
- Quarterly meetings to raise the profile of PCP in
Childrens services - Meetings with Local Education Authority
- Looking at developing person centred reviews in
schools - (Two meetings with third planned for November)
- Have been contacting other authorities to find
out about PC Reviews - Looking into training resources
- Have recieved good feedback from head teachers
about PCP and will arrange for Pete and Claire to
attend Special Heads Conference.
23Whats Happening Now
- All young people in transition at Rosewood School
will have a person - centred review this year facilitated by Phil,
Claire, Mandy or - Myself. (9 young people).
- Phil has agreed to facilitate some of transition
reviews for young - people at Great Oaks School and work with them to
look at ways of - overcoming such issues as time constraints and
staff cover. - We are meeting with the Head Teacher at Cedar
School tomorrow
