Improving Head Start Flu Vaccination Levels

• The CDC AAP recommended in 2008-09 annual flu
  vaccination for ALL children 6 months through 18
  years of age.
• The CDC, AAP, CDPH Immunization branch highly
  encouraged the launch of school-based flu
  clinics.
• In 2008 CDPH made Flumist vaccine readily
  available.

Head Start and Flu Immunization

• Flu immunization is not a State required
  immunization for child care entry.
• Neither ChildPlus nor the Blue Card captures
  flu immunization.
• When discussing immunizations with parents it is
  easy to overlook the recommendation for annual
  flu vaccine.

Barriers to Flu Immunization

• Flu vaccine is NOT a required vaccine.
• Short window for obtaining flu vaccine.
• Stocking of flu vaccine is problematic.
• The scheduling for well child checks and flu
  vaccine availability are NOT in synch.
• Peak illness season creates a scheduling
  difficulty for providers to administer flu
  vaccine.

Flu Clinics in Head Start

• Alameda County Public Health Immunization Program
  partnered with Oakland Head Start and the Samuel
  Merritt University School of Nursing.
• Flu clinics were held at 12 Oakland Head Start
  centers.

• 296 children out of 777 were vaccinated- 38.
• of Flumist Vaccine Administered
• 90 of the vaccinations administered were
  Flumist .

• Vaccine was supplied at no cost to low income
  children via Vaccine For Children Program (VFC).
• Issued standard procedures for special event flu
  clinics.
• Accounted for ALL vaccine usage.
• Assured proper storage, handling, and
  administration of vaccine by nursing students.

• Registry provided flu immunization status for
  Head Start.
• Registry algorithm determined the need for 1 or 2
  doses.
• Flagged contraindications to giving flu vaccine.
• Updated childs record in the Registry
• Copy of updated immunization record given to
  parent.

Role of The School of Nursing

• Screening form reviewed by instructor nursing
  student.
• Nursing students, under supervision of their
  Clinical Instructor, administered flu vaccine
  intranasal or via injection.
• Charted vaccine administration.
• Made referral for 2nd flu vaccine if needed.

• Provide education to Head Start staff for
  enthusiastic buy-in.
• Start your planning organizing before the
  school year ends.

Lessons Learned

• Consent and screening forms to be distributed and
  collected in advance of the scheduled clinic.
• Include Registry consent in the information
  packets given to parents at enrollment Parent
  Orientation meetings.
• Record of immunization will be entered into the
  Immunization Registry in real time.
