Accounting Form
- Starting today, a personalized bar-coded version
will come attached to every lead you download for
your convenience - A CAF form (either lead or non-lead version) is
required for every household you write in.
(Multiple apps in the same household only need
one CAF form) - The form with the bar-code at the top is for
sales that directly resulted from the lead it is
attached to. - The non-lead form without the barcode at the top
can be used for all other non-lead sales. (more
on that later)

- The bar-code at the top matches the customer
number of the lead. (Thats how we track the
sale back to the lead.)

you take a blank ERS letter in the home, and have
the client complete it. - We will have a personalized, blank ERS letter
available for download on the MyLeads page - Before you submit the app, transcribe the
information from the blank letter to this form,
and KIT Marketing will mail out the letter on
your behalf! - No licking
- No stamping
- No trips to the post office

simply tell us where the sale came from. - If the sale is from the result of an ERS letter,
or an existing client, enter a phone number in
the field provided and you will get credit for
making a sale through ERS. - Note We cant give you recognition for ERS
sales if you mail your own ERS letters. - Keep a stack of these in your briefcase.

Response System Form (Non-Lead)
- Exactly the same as the bar-coded version, except
for the lead bar-code at the top. - Feel free to use as many of these as needed for
your ERS letters. As long as the forms are all
included in the same stack for the same
household, well know how to handle the extra
sheets. - Keep a stack of these in your briefcase

- When person living at the address listed on lead
purchases a policy - Only one form required per household
