Who is Eligible

• Active AFOS member
• Active AOA member
• Junior nominee
• lt10 years service or
• lt10 years from graduation (OD)
• Senior nominee
• individuals not qualifying for Jr.
  award

Member Definition

• An active AFOS and AOA member is a member who is
  in good standing. This means the individual has
  already applied for membership to AFOS and AOA
  and dues are paid in full prior to the time of
  submission of the nomination to the awards
  committee.

Who may Nominate

• Members senior officer or supervisor
• Anyone who knows the members qualifications

Nomination

• All accomplishments including but not limited to
• military career
• involvement in AFOS/AOA
• other organizational involvement
• service to optometry
• service to political subdivisions
• service to the public

• AOA Award Guideline
• Junior Optometrist of the Year
• Optometrist of the Year Award

Submission Process

• Deadline November 15
• E-mail submissions to Dr. Carol Rymer

Selection Process

• The Judging panel
• 5 active AFOS Optometrists
• Members of AFOS for minimum of 2 years
• Multiple branches of service represented

Notification

• Recipients and nominees will be notified the
  first week in January to allow for planning in
  order to attend the annual meeting

Presentation of Award

• AFOS Annual Meeting
• Held in conjunction with Southern Educational
  Congress in Optometry (SECO)
• Atlanta, Georgia
• February following nomination deadline
