Title: Role of Homeopathy in Clinical Practice

1
Role of Homeopathy in Clinical Practice
  • Dr D P Rastogi
  • DMS., MBS,.MD Hom, DF Hom

2
Materia Medica and Repertory-
  • Are like two sides of the coin.
  • It is a fact that teaching and use of repertory
was long neglected in Indian homeopathic
    colleges.
  • As a result complete reliance was placed on
learning of the Materia Medica for making the
    prescription

3
Historical---- contd
  • However the scenario changed particularly after
the CCH laid down the syllabus of repertory.
  • The first homeopathic repertory in India was made
by Father Muller in 1885. Alphabetical
  • Late Dr LD Dhawale was in correspondence with Dr
CM Boger and appreciated the Boger-
    Boenninghausen approach.

4
Historical ---contd
  • Dr Bhanu Desai popularised Boenninghausens
repertory through his book How to find the
    simillimum with Boger- Boenninghausens
    repertory.
  • Dr.Bogers repertory portion included in the
Synoptic key formed the base for Dr Phataks
    Alphabetical repertory.
  • Late Dr Sankaran used repertory a lot and made
popular the subject through his lectures.

5
Historical---contd
  • One of the original person who made tremendous
contribution to the subject has been legendary Dr
    Jugal Kishore.
  • Finding printing errors and making corrections in
Kents repertory has been Dr R P Patel who has
made rich contribution and published an authentic
    Kents repertory.
  • Credit for giving a popular text book to the
students in the subject has been Dr SK Tewari.

6
Need of the repertory
  • Was felt by Hahnemann him self.
  • And he used Boenninghausens Repertory of the
Antipsoric in his practice besides his own
    repertories.
  • In 1927- 1600 remedies listed in materia medica .
  • In 1955- 2000 remedies are listed out of which
    1500 are proved.
  • In 2005 the number has become more than 4200.
  • Thus to meet the challenge of the exploding
materia medica, the homoeopathic repertory is
    born.

7
NEED OF REPERTORY---cont
  • The repertory analysis clears our vision and
points us to the remedy that will cure the
    patient.
  • However it should be emphasized that Repertory
    suggests Materia Medica decides.
  • Repertory is not intended to replace a diligent
    study of Materia Medica .

8
Use and utility
  • Utility of repertory is realized because it
points out the similar remedies for a given case.
  • However utility of a particular repertory always
depends on our familiarity with the rubrics.
  • Nowadays repertory soft wares have made our task
of finding the rubrics very easy

9
Various approaches
  • There are 3 standard approaches to the repertory
usage namely The Kentian, Boenninghausen and the
    Boger .
  • Repertorium Universalis is now available which
makes it applicable to any of the above
    approaches.
  • However repertory can be used to look up even one
    main symptom of the patient.

10
  • Examples
  • A patient presented the picture of depression. On
asking the cause she narrated that she had given
some money to one of her close relative . But the
    person refused it to return.

11
  • On the other hand he denied point blank that she
    ever gave that money.
  • It is more the deception which is worrying her
    she said.
  • I looked up the rubric Grief deception from and
was able to visualize the leading remedies out of
which I selected Ignatia which completely
    relieved.

12
  • The remedies given in Complete Repertory under
the rubric- Grief from deception are
  • AUR, IGN, LYC and NAT-M
  • Nux-v
  • lach, merc, op, ph-ac, puls, sep , verat

13
A case of Blepharospasm
  • A patient presented blepharospasm of both eyes
which could not be helped with the best of
allopathic treatment. The patient also had
photophobia. The case was repertorized in Knerrs
repertory where Euphrasia was suggested as the
only remedy under Lids- spasms with photophobia.
The case was cured with Euphrasia.

Case of genital warts
  • A lady accompanied one of my patient and
    presented problem of genital warts.
  • There was no other symptom except itching.
  • Her family physician has ruled out Hiv infection
    by elisa test.
  • I could not discover any particular desire or
aversion for food nor could I find anything
characteristic in the mind to provide any
    guidance for prescription.

16
  • I looked up Knerrs repertory and found the
solitary remedy Sabina mentioned under Genital
warts- itching. I prescribed Sabina 0/1 to be
taken 4 times a day for 15 days.
  • After 15 days the report came that most of the
    warts have fallen.

Cina
  • Treating cough in children is often
problematic as it is both annoying to the patient
as well as to the parents.
  • I have often found concomitants given in the
    chapter of Boger-Boenninghausen repertory very
helpful in picking up the remedy.
  • For Cina the repeatedly verified concomitant has
    been cough with disagreeable behavior.

Conium
  • A very senior executive was recommended to see me
for his foot drop following some injury to the
    back
  • A quick repertorization on the following rubrics
    was done
  • Lower Extremities foot
  • Lower Extremities drags the feet
  • Back Lumbar region

21
  • Feet ,cant place them on ground
  • On the basis of the above rubrics using BBCR
Conium appeared to be the remedy, covering all
rubrics which was given in 0/1 every 2 hours. On
the third day improvement was reported and more
medicine was asked for. At the end of 7 days it
was noticed that there was no more dragging of
    the foot.

22
More Examples
  • I had a patient whose main complaint was vertigo,
it was worse lying on right side
  • I looked up the vertigo chapter and found the
    following entry

  • Thuja came out very prominently.
  • It was given in 0/1 4 times daily for a month.
  • Improvement was reported in both back pain and
    vitiligo spots around the mouth
  • The same medicine is being continued with
    progressive improvement.

42
  • Repertorisation has been recognized as one of the
important strategies to find the most similar
remedies for a given case .
  • Computer repertory soft wares have given the
option of using several repertories for a given
    case.
  • However, Boger-Boenninghausen repertory BBCR
has been found reliable for working out cases
of varied pathology and has been an effective
homeopathic tool which has stood test of time.

43
  • In the Boger Boenninghausen Characteristics and
repertory BBCR Boger brought together all of
Boenninghausens different repertories as well as
a large number of additions from the Aphorisms of
    Hippocrates.
  • He also added several remedies not proved in
    Boenninghausen time.
  • In this repertory Boger goes a long way towards
correcting the major criticism of Boenninghausen
plan by giving modalities in particular
separately from modalities in general, although
the general rubrics remain the same.

44
  • This introduces some of the precision of the
Kents repertory without losing the essential
    flexibility of the Boenninghausen plan.
  • In fact it is a bridge between very flexible
    Boenninghausen and very rigid Kent.
  • He made a serious attempt to unite analysis and
synthesis in each rubric, expressed usually in
    only one or two words.
  • In this way, the avoidable clumsiness inherent in
repertory use is streamlined to great degree.

45
  • One should have all major repertory soft wares on
computer and have an open mind for their use and
use them all as per need.
  • However BBCR has proved more useful in all type
of cases in my experience and this is the most
    often used by me.
  • I describe some more cases from my practice to
demonstrate the role of repertory in clinical
    practice.

46
A case of Ankylosing Spondylitis
  • Mr. AP aged 20yrs, a medical student approached
for homeopathic treatment after exhausting all
    options of conventional medicine.
  • As he was a medical student at the medical
college best of care was provided to him without
    any resolution of his problem..

47
  • He complained of severe pain in left hip
radiating down the back of the knee.

Selection of the remedy
  • There were 3 remedies strongly contesting namely
Nux vom, Sulphur and Ars album.
  • As patient was chilly Sulphur was not considered.
Desire for company went against Nux vom which has
lt from company. Anxiety and desire for company
went in favor of Ars alb 5marks
  • Ars alb LM 1 was given 4 times a day and was
raised serially to LM 4 over a period of six
    months.
  • The case was completely cured and continued his
studies with a promise to take up homeopathy.

54
A case of Rheumatoid arthritis
  • Miss NS aged 23 years was diagnosed a case of
    Rheumatoid arthritis.
  • As usual homeopathic treatment was not her first
choice. She approached me after she failed to get
    relief under allopathy.
  • Her left wrist , dorsum of foot and elbow were
    affected.
  • She was greatly aggravated in cold.

55
  • She had a desire for sweets, sour and condiments.

59
Remedy selection
  • Sulphur and Rhus tox were the two remedies which
    were very close.
  • As Rhus tox had higher grade in aggravation from
cold than Sulphur, it was decided to be given.
  • It was given in LM 1 and resulted in increasing
    relief.
  • The case was finally closed with Sulphur LM1

60
A case of Ankylosing arthritis
  • Mr. PSK aged 24 yrs, a soft ware engineer
    consulted me for treatment.
  • He was undergoing treatment at the best treatment
center in New Delhi without relief.
  • His knee joints and back , both dorsal and lumbar
    regions were affected.
  • He was diagnosed to be suffering from Ankylosing
    spondylitis.

61
Other symptoms of the case
  • He had a desire for salty things.
  • Desired cold ,icy liquids.
  • Had desire for juicy things.
  • He had great aggravation at the change of
    weather.
  • He revealed a tendency of procrastination.

65
Remedy selection
  • Phos acid covered most of the rubrics including
    modalities and the mental symptom
    procrastination.
  • Therefore it was selected and administered in LM
    1
  • It was continued and brought quick overall relief
    to the patient.
  • An intercurrent dose of Sulphur LM 1 was given
    every 2 weeks.

66
Discussion
  • The patient did not want to stop the allopathic
medication and therefore simultaneous use of
    homeopathic medication continued.
  • The patient continues well and had no pains and
has no difficulty in ascending stairs.
  • His ESR westergren has come down to 10mm/1st
hour from 80mm/1st hour during 5 months of
    treatment.

67
Eizayagas repertory
  • Some repertories more particularly clinical and
Alphabetical are very useful in clinical
    practice.
  • In this category mention may be made of O E
    Boericke, Murphy and Eizayagas repertories.
  • I would like to give a glimpse of Eizayagas
