Texas Medicaid Enrollment

• Two options for enrollment
• Paper application
• Online application
• Paper application requires complete application
  for each site
• Online application requires account
  set-up/activation

Online EnrollmentAccount Set-up Activation

• www.tmhp.com and select activate my account
• Select Create a provider/vendor administrator
  account
• On next page, select Create a provider/vendor
  administrator account again
• In drop down box, select provider enrollment
• The next page requests specific information
  (name, email, password, etc.)
• After completing this information, click on
  Agree to Terms and then Create Provider
  Administrator

Online EnrollmentBegin Application

• Click My Account link located in blue bar at
  the top of the page
• Log on using the account you just established and
  activated
• Click Administer a provider enrollment
  transaction
• Begin enrollment process by selecting
  Traditional Medicaid
• On next page (Provider Type Identification)
• Are you enrolled in Medicare select No
• Applicant is enrolling as select Facility
• Enter NPI and click Verify NPI
• Traditional Services
• select Federally Qualified Health Center for
  enrollment of the parent site
• select Federally Qualified Satellite for
  enrollment of additional sites
• Taxonomy is requested on next page 261QF0400X

• Pages 2-14 must be completed
• All signatures must be original no copies or
  facsimile stamps are accepted
• Documentation to submit with application
• HRSA Notice of Grant Award
• Articles of Incorporation (if a corporation)
• List of all principals (as defined on page 9.4)
• Notice from your state agency/administrator of
  current PPS rate
