Transition from the BCBS of Minnesota program
  • The previous program name was BluePrint for
    Health care support.
  • The previous phone number was 888-264-1744. This
    number will be answered through 3/31/09 by
    Healthways clinicians with information about
    program offerings for South Central Service
    Cooperative members.
  • A transition letter was sent around 12/15/08 to
    previously active disease management program
    participants explaining the change to the program
    name and phone number.

  • The Member Portal is a new customized website
    for delivering health-related material to SCSC
    members via the Internet that is condition
    specific. Additionally, the Member portal
    features the Healthwise Knowledgebase, including
    interactive tools, searchable health topics, drug
    references and other related information.
