  • Experience has shown CAS staff that it is only
    when the dust settles that carers realise the
    enormity of the task ahead of them.

The money..
  • A grant from NSW Health Carers Program
  • Established in 2002 by the Minister for Health
    Ageing and the Minister for Disability Services
  • The Program provides a range of support for
    carers

The model.
  • To offer information, resources, education and
    phone support to first time carers following the
    discharge of the person they care for from an
    acute or sub-acute hospital ward, in a timely and
    targeted manner
  • Face to face and telephone
