- 1. Joint Operations Each Service will record
deaths, injuries, illnesses to their own service
member regardless of the cause. Friendly fire
incidents will be included. - 2. Civilian Severity Rates DFAS/DMDC will
provide from Civilian lost workday timecard data. - Civilian Frequency Rates CPMS will provide from
civilian workers' comp cases. MWR rates will be
provided. - Top 40 Installations Highest lost day
installations. DMDC will update quarterly. - 5. Military Frequency and Severity Rates DMSS
will provide from military medical treatment
data. - Aviation Service Safety Centers will provide
Aircraft Class A and B data (number of accidents,
rates destroyed aircraft). - UAV numbers and rates. Service Safety Centers
will provide Aircraft Class A and B data (number
of accidents, rates destroyed aircraft). - Top Aviation Brigades/Wings Highest losses by
