Why contact the nurse?
  • If your child has (a) Special Medical
    Condition(s) such as vision hearing loss,
    diabetes, seizures, asthma. Severe allergies may
    require the need for medication or medical
    treatments during the school day.
  • The school office will provide the needed forms.
    Consultation with your school nurse may be
    helpful.

What about medications at school?
  • The only drugs allowed on campus are prescribed
    by a physician or your Health Care Provider and
    provided by parents / guardians.
  • If your child needs medication, you and your
    physician or HCP will complete the medication
    authorization form.
  • OTC (over the counter) and prescription
    medications must have prior medical approval
    before coming on campus. Adults must transport
    all controlled substances.

  • Immunization Requirements prior to school
    entrance.
  • 4 DPT (4th one after 4th birthday) DT Booster
    needed every 10 years.
  • 3 Polio (3rd after 4th birthday)
  • 2 MMR (1yr and after 15 ms)
  • 3 HBV ( 1, 2 -1m, 3 -6ms)
  • Chickenpox disease or varicella vaccine after 12
    months of age
