New Hampshire Women, Diabetes, and Depression
Grant

• Partnership of the Southern and Northern New
  Hampshire Area Health Education Centers (NH AHEC)
  and the New Hampshire Diabetes Education
  Program (NHDEP) and their community partners.
  
• Funded by the Womens Health Council of the
  National Association of Chronic Disease
  Directors.
• Raise health care professionals awareness of
  the coexistence of diabetes and depression and
  to improve identification of depression among
  women with diabetes through focused
  educational interventions.

Workplan

• Design a 1-hour training module addressing the
  risk factors for depression, interrelationship
  between diabetes and depression, vulnerability of
  specific populations to depression, screening
  tools and diagnostic procedures, and referral
  procedures, including resources on WHC website.
• Publicize and arrange logistics to offer this
  training at 8-12 sites
• Disseminate written information on diabetes and
  depression
• Perform an audit of current practice within the
  NHDEP site network with regard to screening,
  diagnosis, and treatment of diabetes among the
  female diabetic patient population.
• Perform a follow-up audit of the NHDEP clinical
  site network to measure whether depression
  screening and referral have increased for this
  population.

Progress to Date

• A planning committee was convened
• In the fall, the group met to identify learning
  objectives and content for the one-hour training
  module.
• From January through March we offered 6 programs
  to 93 Ammonoosuc health professionals. We have
  offered three programs in northern NH, one in the
  central part of the state and two in the southern
  counties.

Progress to Date (contd)

• Additional trainings are scheduled for April, May
  and June at 5 community health centers. In April
  we will also be offering the program as part of
  the Rural Health Symposium.
• In May we will be highlighting this educational
  offering at our spring NH Diabetes Coalition
  meeting.
• There is significant demand for this program and
  we are trying to leverage other funds to continue
  to offer the program at additional locations.

Goal

• Develop and offer training on diabetes and
  depression in 8-12 sites reaching approximately
  220 individuals.
• State the risk factors for depression
• Discuss the interrelationship between diabetes
  and depression
• Identify the vulnerability of specific
  populations to depression
• Discuss screening tools, diagnostic and referral
  procedures.
