- We serve all of Suffolk County with
- nine sites located in the following areas
Wyandanch, Amityville, Bay Shore, Patchogue,
Riverhead, Greenport, Islip and Montauk. Our
HIV/AIDS Services also provide assistance to
Nassau residents.

7Comprehensive Case Management
- Our comprehensive case management program is
regulated by the NYS DOH AIDS Institute, and
provides family-centered, community-based
psycho-social case management services for
Medicaid-eligible HIV-infected individuals
throughout Nassau and Suffolk Counties. The
program utilizes a team model approach for
service delivery, provided by case managers and
paraprofessionals who work together to provide
comprehensive, intensive services.

- The program is designed for HIV persons with a
variety of service needs, who often require
frequent contact with care providers and have had
difficulty gaining access to care and retaining
services. The goals of the program are to
increase universal access to services, promote
early intervention, prevent or delay
institutionalization, and foster independence and
self-sufficiency.
9Supportive Case Management
- Our Supportive Case Management program is also
funded regulated by the NYS DOH AIDS Institute,
and provides family-centered, community-based
psycho-social case management services for
HIV-infected individuals, regardless of their
Medicaid status or income. This program is
administered only in Suffolk County. The
program utilizes a team model approach, provided
by case managers and paraprofessionals who work
together to provide supportive services.

services. This program also provides services to
individuals who are uninsured. The goals of the
program are to increase universal access to
services, promote early intervention, prevent or
delay institutionalization, and foster
independence and self-sufficiency, however they
are accomplished with less intensity.
11Transportation Services
- Our transportation program provides free
transportation, via taxi and ambulate service,
to HIV-infected individuals residing in Suffolk
County. The goal of the program is to operate a
transportation program that provides a convent
service to clients who are living with HIV/AIDS
in order to access health care routinely or on
an emergency basis in Suffolk County. - To be eligible, proof of HIV status must be
submitted.

19What is MSC?
- Medicaid Service Coordination is a service
available for all Medicaid-eligible adults
diagnosed with a developmental disability. MSC
provides these individuals and their families
with the linkage and coordination of all
available services and entitlements. A highly
trained and qualified Service Coordinator works
with the individual and their family to first
develop an Individualized Service Plan (ISP),
which serves as a Life Plan to help identify
life goals, wishes and desires. It is then the
job of the Service Coordinator to help the
individual in realizing these goals by linking
them to needed and desired services. EOC of
Suffolk. has been providing MSC for many years
and is a proven leader in the field of providing
services to individuals with developmental
disabilities.
20Who is eligible for MSC?
- A person must meet all of the following
criteria - Must be enrolled in Medicaid.
- Must have a documented diagnosis of mental
retardation or developmental disability. - Must demonstrate a need for ongoing and
comprehensive service coordination. - Must choose to receive MSC.
- Must not permanently live in an ICF,
Developmental Center, Psychiatric Hospital, Small
Residential Unit, Nursing Facility, Hospital, or
any other Medicaid funded institutional setting
that provides service coordination. - Must not be enrolled in another comprehensive
Medicaid long- term care Service Coordination
(e.g. Long-term home Health Care).
21Residential Habilitation Services
- People with special needs require specialized
supports to enable them to live successfully in
their community. In-Home Residential Habilitation
is a Medicaid waiver program that addresses the
needs of individuals with developmental
disabilities living at home. People with special
needs often need training and assistance in
achieving fulfilling their roles as part of a
family and within the community. A trained staff
person works with individuals to assist in
accomplishing the consumers desired goals.
Staffs work with the individual to increases
skills and facilitate their independence,
productivity, and integration into the community.

22What Is Respite?
- Respite Care Service is a temporary relief option
available for adults and children with a primary
diagnosis of mental retardation. The primary
purpose of respite service is to give relief to
families and caregivers from the extraordinary
and intensive demands of providing
round-the-clock care. EOCs Respite and
Recreation Programs are designed to enhance the
quality of life for individuals with
developmental disabilities. EOC provides this
service by involving individuals in activities
that are both fun and educational. - EOCs Respite program provides recreational
supports in the Suffolk area of Long Island. The
program runs on Fridays and Saturdays all year
round. The focus of this program is on
development of independent leisure activities.
Participants are individuals with mental
retardation who have independent living skills.
All activities are planned according to the
group's capabilities and interests and include
recreational activities such as trips to the
sporting events, Nassau Coliseum, amusement
parks, bowling, plays and much more.
23Respite (Summer Fiesta)
- Summer Fiesta is a summer camp alternative for
individual with special needs. This mini-travel
camp operates 2 weeks in the summer months. - EOC believes that group travel is an ideal way
to experience independence, improvement social
skills, enhance self esteem in a secure and
exciting environment. The participants of Summer
Fiesta are high functioning individual ranging
from the ages of 21 to 60. These individuals have
varying developmental disabilities, which
requires them to need more supervision and
guidance while on trips.
24ISS Grants Individualized Supports Services
- ISS is higher level of residential opportunity
provided to individuals with a developmental
disability. This grant offers individuals who
need minimal supports the opportunity to live
more independent lives in their community. The
ISS Grant offers financial support for individual
who possess the skills and ability to live
independently. The monthly subsidy is based on
personal living expenses and is used to
supplement their income which provides an
opportunity to live in their own apartment in the
community. - This program also provides residential
habilitation services within an individuals
private home or living arrangement. One of the
main goals of this program is to increase an
individuals daily living skills while supporting
the family. This program encourages some
individuals to strive to live independently in
the community with minimal staff support.

26WEED AND SEED PROGRAM
- Our Weed Seed program is a Department of
Justice initiative which employs, strategies that
are community based in a comprehensive
multi-agency approach to all enforcement, crime
prevention and community revitalization that
affects the quality of life in the Wyandanch
community.
