Purpose

• Established Under Article 2B Section 23-a
• Registry Is Distributed By Human Service Agencies
  As Part Of Their Normal Intake Process.
• Agencies Involved But Not Limited To Public
  Health, Office For The Aging, Meals On Wheels,
  Hospice, Home Health Care, Lifeline
• Forms Are Sent To The Emergency Management Office
  Where An Office Volunteer Enters The Data Into
  The Database.
