Background
  • H-SIPs submitted to date
  • Helped the LHIN identify local health system gaps
    and informed the development of strategic plans
  • Remain a valuable source of innovative ideas for
    health system improvement
  • Resulted in projects funded by Toronto Central
    LHIN and executed by Health Service Providers
  • Recently we reviewed feedback from Health Service
    Providers and the Toronto Central LHIN team to
    determine how we can best utilize the H-SIP
    process going forward
  • Result A re-designed H-SIP process and form to
    promote clarity for both Health Service Providers
    and Toronto Central LHIN

How are H-SIPs used at the Toronto Central LHIN?
  • At the Toronto Central LHIN the H-SIP form
    is the sole venue available for Health Service
    Providers to articulate
  • A request for funding to implement a new health
    service that the organization has not previously
    provided
  • A request for funding to expand an existing
    service
  • A request for funding to improve efficiency or
    reduce barriers to care
  • An informal notification of voluntary integration
    in advance of formal notice
  • Before providing a formal notice of voluntary
    integration, Health Service Providers must first
    submit an informal notice of voluntary
    integration using the H-SIP form. Following this
    submission, Toronto Central LHIN staff will
    contact the submitting Health Service Providers
    and instruct them to either proceed to a formal
    notice of voluntary integration or to supply the
    LHIN with additional information.

Key Considerations for Submitters
  • Before submitting an H-SIP, Health Service
    Providers (HSP) should make
  • note of the following
  • The Toronto Central LHIN has a limited amount of
    discretionary funding at its disposal, most of
    which is one-time in nature
  • HSPs should be prepared to describe how their
    proposal will be sustained after LHIN funding is
    complete
  • When significant multi-year funding becomes
    available, the Toronto Central LHIN may make note
    of it through a targeted call for proposals
  • The Toronto Central LHIN gives particular
    attention to H-SIPs that align with the LHINs
    and the MoHLTCs priorities.
  • HSPs should be aware that their proposals may be
    shared with potential collaborators

The H-SIP Process at the Toronto Central LHIN
  • H-SIP Submissions
  • H-SIPs can be submitted at any time throughout
    the year. The form can be downloaded from the
    Toronto Central LHIN website and sent in via
    email.
  • Acknowledgement of Receipt
  • HSPs will be sent a letter of acknowledgement
    specifying the date their H-SIP was received.
    HSPs must take note that their proposals will
    expire one year from this date.
  • HSPs are welcome to resubmit expired proposals
    with updated information.
  • H-SIPs submitted prior to September 2008 will
    expire September 1, 2009 and do not need to
    re-submit using the new form.
  • H-SIP Reviews
  • During the fiscal year (April 1st ? March 31st)
    the Toronto Central LHIN performs two scheduled
    reviews. All H-SIPS received prior to a scheduled
    review date will be considered during that
    review.
  • All H-SIPs remain in funding consideration until
    their expiration date, and are considered during
    all reviews, as appropriate.
  • Additional reviews may be conducted at any point
    throughout the year should a new funding source
    or urgent requirement deem it necessary.

Key Considerations used by the Toronto Central
LHIN
  • In evaluating H-SIPs, the Toronto Central LHIN
    will consider the following
  • Does the proposal align with the LHINs and
    MoHLTCs current priority areas?
  • Does the proposal support timely, measurable,
    well-defined outcomes?
  • Have consumers, residents and other stakeholders
    within the community been engaged in identifying
    this proposal as necessary and appropriate to
    meet community needs?
  • Are the submitted cost projections reasonable?
  • Does the proposal support health service delivery
    sustainability?
  • Does the proposal demonstrate strong partnerships
    and collaboration?
  • Is the proposal evidence-based?
  • Does the proposal target improvements in LHIN
    priority neighborhoods?
  • Are the submitting HSPs meeting the requirements
    set out in their accountability agreement with
    the Toronto Central LHIN, specifically
    maintaining a balanced budget?
