Upload Common Problems/Errors

• Incorrect Month If your data is not entered in
  the row for the month the data is due, you will
  receive the message The Excel Key Measures
  Spreadsheet you have attempted to upload does not
  contain data in the correct month's row. Data
  from reports run on the last working day of the
  month are entered on the next month's row. Please
  adjust your data into the correct row and upload
  again or contact your Cluster IS Specialist for
  further assistance. For example, reports due
  October 1, 2004, must have data entered in the
  Oct 04 row on the excel template.

UploadCommon Problems/Errors

• Incorrect Specialty A Specialty file not of the
  type allowed will generate the error message
  example Your excel spreadsheet for Diabetes
  could not be uploaded. Data in file indicates it
  is a Cardio file. Please contact your IS
  Specialist. Remaining files on page have been
  ignored and have not been uploaded. Please
  correct error or upload files without file
  causing error.
• Outdated Template Spreadsheet Uploading an
  out-of-date template spreadsheet will generate an
  error message that begins Your excel
  spreadsheet for Depression could not be uploaded.

• Gives the user the ability to directly send an
  email to the development team in case of a
  problem
• Lets the user define the Priority Level to be
  assigned to the problem, depending on how it
