UA offering Open Enrollment !
  • UA is offering open enrollment for Medical
    Dental plans through Dec 15, 2006.
  • UA does not offer annual enrollment every year.
  • Take advantage now! It may be years before we
    have another open enrollment.
  • Only other option to add coverage is if you are
    within 31 days of a qualified family change (such
    as marriage, divorce, or birth of child also
    spouses loss of coverage if he/she quits
    working).

Open Enrollment through Dec 15
  • In additional to Medical Dental, Open
    Enrollment is also being offered for
  • Flexible spending accounts
  • Premium conversion
  • NEW vision plan
  • NEW critical illness plan
  • Long term care plan
  • Basically, open enrollment is underway for all
    insurance plans except Life and LTD.
  • Your new benefits would take effect January 1,
    2007.
  • Mark December 15 on your calendar so you dont
    miss the deadline!

What else is new besides Open Enrollment?
  • Optional LTD rates increasing by 3.5 on 1/1/2007
  • Current highest possible rate of 36.67 (for
    someone who makes 100,000 or more) is increasing
    to 37.95.
  • Minor plan changes to Long Term Disability
  • Tuition discount forms being accepted now for
    Spring 2007 semester
  • Complete Personal Health Appraisal by Dec 15, get
    100 or 200 premium discount in February

Personal Health Appraisal (PHA) What is it?
  • Part of new UA ChooseWell wellness preventive
    care program available to medical plan enrollees
  • Tool to assess your risk for a health problem
  • QCare nurses can help you with a health action
    plan also help you to understand your medical
    and prescription benefits.
  • Only applies to employees spouses enrolled in
    medical as of October 1, 2006
  • Premium discount is the carrot.

Personal Health Appraisal (PHA) What you need to
do
  • Log into the secure website of QualChoice/QCare
    and complete the PHA before Dec 15.
  • Instructions are on our website.
  • Paper form available in Human Resources for
    employees without access to a computer

Medical insuranceWhats new?
  • No rate changes
  • No benefit changes
  • However, Drug Formulary may be updated to address
    new drugs added to the market.
  • Go to www.uams.edu/ohr (human resources home
    page) and click on Benefits, then Medical to look
    at a benefit summary, comparison of plans, rates,
    etc.
  • Open enrollment information mailed to home and
    campus addresses

  • Complete QualChoice form if you want to enroll.
    Also complete form if you want to add any family
    members.
  • Current Participants
  • No action required if you dont want to make any
    changes
  • Complete Open Election form if you are already
    enrolled in Medical but want to change plans
    from Classic to Point of Service, or vice versa.
  • Review eligibility of family members. Drop
    coverage NOW for any ineligible ex-spouses or
    children.
  • Take the Personal Health Appraisal!

  • Classic Managed Care Plan
  • Similar to an HMO no benefits outside UA
    QualChoice network
  • Lower premium
  • Reduced mental health benefits (10 visits per
    year), no TMJ coverage
  • Point of Service Plan
  • Same in-network benefits as Classic, but also
    pays some if you go out-of-network (after
    separate deductible is satisfied)
  • Higher premium because of the additional
    benefits, flexibility
  • Best mental health benefits (30 visits per year),
    partial TMJ coverage
  • Recommended if you have a child attending school
    out of state

  • No rate changes
  • No benefit changes
  • Go to www.uams.edu/ohr and click on Benefits,
    then Dental to look at a dental benefit summary
    and rates.
  • Open enrollment information mailed to home and
    campus addresses

Dental insuranceWhat you need to do
  • Open Enrollment
  • Complete Delta form if you want to enroll. Also
    complete form if you want to add any family
    members.
  • Current Participants
  • No action required of you dont want to make any
    changes
  • Review eligibility of family members. Drop
    coverage NOW for any ineligible ex-spouses or
    children.
  • Add children age 2 or older.

Section 125 Premium ConversionWhat you need to do
  • Nothing if you dont want to change how your
    Medical and Dental premiums are currently
    deducted from your paycheck.
  • If you want to change to have your premiums
    deducted on a pre-tax basis, complete the
    pre-tax form.
  • Or vice versa --you want to change from pre-tax
    to post-tax fill out the same form.
  • Changes take effect first paycheck of 2007.
  • FYI, most employees pre-tax to reduce their
    taxes. Risk is that youre locked into that
    deduction and coverage for the year unless youre
    within 31 days of a qualified event.

Flexible Spending AccountsWhats new?
  • Nothing new. Maximum amounts you can contribute
    stay the same 4,000 health care and 5,000
    dependent care.
  • 2007 will be 2nd year for the Benefit Card
    (applies to those with a health care FSA).
  • ALWAYS keep your receipts when you swipe your
    card. Wait for Conexis to request them. IRS
    rules dictate this.
  • Benefit Card use is voluntary. You can opt
    instead to file a paper claim.

Flexible Spending AccountsWhat you need to do
  • Consider if you will have sufficient
    out-of-pocket medical costs in 2007 to do a
    health care FSA.
  • Consider if you will have daycare costs in 2007
    to do a dependent care FSA.
  • If so and youre comfortable with the FSA claim
    process, enroll on-line at www.conexis.org
    between Nov 15 and Dec 15.
  • Current participants re-enroll if you want to
    participate next year.
  • Go to www.uams.edu/ohr and click on 2007 Open
    Enrollment for details and link to Conexis
    website (where you enroll on-line).

Vision planNew voluntary benefit
  • Open Enrollment through December 15
  • Enroll through the carrier, Spectera
  • Monthly premium between 5.89 (employee only) and
    18.07 (family)
  • Pre-tax premium deduction
  • You must keep coverage for entire year, no
    changes.
  • Benefit highlights if you see a Spectera provider
  • You pay 10 for annual eye exam
  • You pay 20 for eyeglasses (once every 2 years)
    or for contacts (once every year)

  • If you are setting up a 2007 health care FSA,
    only factor in 30 out-of-pocket costs for

  • 20 materials co-pay to cover eyeglasses or
    contacts

Critical illness planNew voluntary benefit
  • Open Enrollment through December 15
  • Enroll through the carrier, MetLife
  • Benefit highlights
  • Entire policy (10,000 to 100,000) generally
    paid out in full upon diagnosis. You spend it
    as you like.
  • Covers cancer, heart attack, stroke, major organ
    transplant, kidney failure, coronary artery
    bypass graft
  • Replaces Transamerica cancer policy

Critical illness planWhat you need to do
  • Consider if this plan will benefit you.
  • If so, enroll before December 15 deadline.
  • Enroll on-line at www.metlife.com/mybenefits,
  • Or fill out form that MetLife sends to you.
  • If you have Transamerica cancer policy, decide if
    you want to
  • Replace that with MetLife critical illness
  • Keep it and also sign up for MetLife critical
    illness (2 policies are ok)
  • Keep Transamerica cancer and pay premium directly
    to Transamerica starting in Jan or Feb 2007 (they
    will send you direct bill info).

Long term care planWhats new?
  • No changes to benefits offered by CNA to new
    employees
  • But special Open Enrollment is being offered to
    current employees, through December 15.
  • Current participants may be given the opportunity
    through CNA to upgrade their coverage during Open
    Enrollment.

  • Enroll on-line at www.ltcbenefits.com by December
    15.
  • Or send in form that CNA sends to you.
  • Current Participants
  • No action required of you dont want to make any
    changes
  • Consider upgrading or adding automatic benefit
    increase option as offered by CNA through the
    mail.
