- Safety Assessment Document
- The SAD for LCLS Injector Commissioning has been
approved and is on file. - Identifies the hazards and risks in the Injector
- Defines the mitigation of those risks
- It received internal review and review from
safety committee chairs and safety officers. - Final review and acceptance was through the
Directors office.

- Accelerator Division Procedures
- The Beam Authorization Sheet (BAS) is a
prerequisite Control Room document for beam
operations. - The BAS requires formal approval from Radiation
Physics, Accelerator Division Safety Office
(ADSO) and Accelerator Operations. - Specifies pre-running conditions, running
conditions, and initial checkout requirements. - Pre-running conditions must be signed off before
operations begin. - Assures that the ASE is maintained.

- Example - Running RF
- The intent is to do high power RF processing of
accelerator structures prior to beam operations - An RF-only BAS was written by RP and ADSO.
- Action items from the non-ionizing radiation
safety committee were cleared prior to BAS
sign-off. - Pre-running were signed off PPS was certified,
shielding was checked, ion chambers locations
and trip levels were set, the Gun RF source was
locked off.
