- Identifies children at risk of academic failure
resulting from multiple factors - Involves parents/guardians in identifying causes

- Reviews current functioning and programming for
students identified as vulnerable or critical on
SSAASY - Explores patterns of issues and problems for
students identified as vulnerable or critical - Ensures that students requiring ancillary
academic support receive such support - Identifies needed support services for students

- The school based team referrals come from
- SSAASY
- the discipline matrix (mandated referrals)
- teachers, parents, agencies and peers

- Students identified as possessing risk factors
that indicate a vulnerable or critical academic
status - Students whose behavior requires a referral to
the School Based Team - Students whose behavior, achievement and or
