  Each Yes is worth 10 points. Each No is
  worth 0 points. Add the number of checks under
  Yes and place the total in the appropriate box.

• Purpose The purpose of this assessment is to
  monitor the individual students interactions
  with the game of Footbag and with classmates.
• Equipment Enough hackysacks for each student in
  class.
• Instructions All students will participate in
  games of Circle Kicking. There should be about
  five students in each group. The teacher will
  walk around and observe students until all
  students have been tested. If a student wishes
  to know when they are being tested, the teacher
  will tell them when it is his/her turn.
• Other Students All of the students will play
  games of footbag during the assessment.
• Modifications There will be hackysacks of
  different sizes and colors for those students who
  require them. Each student will receive any
  modification necessary to ensure an equitable
  environment.
