Student Information Form

• Receive a copy of your childs demographics
• Check over for accuracy
• Add any missing information

Verify that all information is correct and return
to school.
Place a five digit code in each of the parental
codes to be made active

match exactly with our JPAMS data.

Choose which status you wish to create a login
for.

Enter just the first and last name exactly like
it is on the Student Enrollment Form.

Enter your five digit PSN code.

If the data does not match with the information
at the school, this message will appear.

If information matches, you will now set up your
login and password along with secret questions.
When done, hit the SUBMIT button.

This screen shows the account was created
successfully. Click on the LOGOUT button which
will return you to the main menu. Click Login to
enter your new user name and password.

If you forget your password, click here to
re-enter a new password if you answer the secret
questions correctly.
