How Do I Get A Blank Job Application?

• There are two ways
• Print one from the PHHS Patriot Exhibition
  website (http//www.patriotexhibition.com)
• Pick one up at a local place of employment

What Items Will I Need to Complete the
Application?

• Social Security Number (you may make one up)
• You should have a SSN however. Ask your parents
  if you have one.
• If you dont have a SSN get one call
  800-772-1213. You cannot get a SSN on-line

• Schools Attended
• Name and address of each
• Dates of attendance
• Special courses taken

What Other Items Will I Need?

• Employment
• Where youve worked
• Name, address, phone number, hourly wage, name of
  supervisor
• Include volunteer work
• Skills you developed at each job
• Reason for leaving job

• References
• Need at least three
• Name, address, phone number
• Avoid using friends and relatives
• Choose teachers, counselors, clergy, business
  people
• Always ask each reference for permission to list
  their names!

What is the First Step to Filling Out a Job
Application?

• Make a photocopy of application you will make
  mistakes!
• Use blue or black ink and write LEGIBLY. Print
  information.
• Read each question or box carefully.

What Should I Avoid Doing?

• No blank spaces
• Dont lie! May result in immediate termination.
• Dont misspell words or use poor grammar
  PROOFREAD!

What if Im Not Really Applying for a Job?

• Purpose of activity learn how to properly
  complete a job application.
• If creating a fictitious job application be
  realistic! Apply for a position suited for a
  teenager.
