What is a Personal Health Record?

• The PHR is The Personal Health Record (PHR) is
  an Internet-based set of tools that allows people
  to access and coordinate their life long health
  information and make appropriate parts of it
  available to those who need it such as their
  healthcare workers.

The Personal Health Record

• PHRs offer an integrated and comprehensive view
  of a a patients health information, including
  information people generate themselves such as
  symptoms and medication use, information from
  doctors such as diagnoses and test results, and
  information from their pharmacies and insurance
  companies.

• Individuals will be able to access their PHRs
  via the Internet, using the PHICON-PHR
  characterized by state-of-the-art security and
  privacy controls, at any time and from any
  location.

• Family members, doctors or nurses can see
  portions of a patients PHR when necessary and
  emergency room staff can retrieve vital
  information from it in an emergency.
• Portions of the PHR can be restricted from view
  if the patient so desires

• The PHICON-PHR relies on state of the art
  technology to store retrievable patient data
  which can be accessed from any where in the world.

• There are new proprietary standards and
  technology idiosyncratic to the PHICON-PHR that
  makes it different from prevailing products in
  the market.
• (These differences will only be disclosed to
  stakeholders with a valid agreement).

  software. The patients health data will be
  centrally maintained and built to prevailing
  health network interoperability standards. (The
  same standards as certified Electronic Medical
  Records)
