WHAT IS AN ICP?
  • An icp is a multidisciplinary outline of
    anticipated care ,placed in an appropriate
    timeframe ,to help a patient with a specific
    condition or set of symptoms to move
    progressively through a clinical experience to
    positive outcomes.

Why are ICPs useful ?
  • By providing explicit standards icps help to
    reduce unnecessary variations in patient care and
    outcomes.
  • They can improve multidisiplinary ( and
    multiagency )communication and collaboration
    empower and inform patients and their carers
    ,and help to meet the requirements of clinical
    governance.
