• 20 for primary care physician (PCP) visit
• 30 for specialist visit
• Coinsurance for some services (e.g., 20 for lab
  work) maximum 1,000
  out-of-pocket cost per calendar year
• No medical deductible or claim forms to file
• For network level of benefits
• Your designated PCP must direct your care
• You must use a network provider
