Patient questionnaires

• The DPD will be sending out at least 25,000
• random questionnaires per month to confirm
• the details reported on FP17s and to gain an
• impression of patient satisfaction
• In addition the DPD can provide each PCT, with
• at least 10 targeted exercises each year (record
• card check/questionnaires/both)
• The results will be reported regularly to PCTs

• The DPD will be requesting records on a
• random basis from at least 10 contracts
• (batches of 10) in each PCT every year.
• In addition the DPD can provide each PCT, with
