Aim of service
  • To enable school aged children and young people
    to achieve their full potential physically,
    socially and emotionally.

Service Structure
  • Service provided by Walsall teaching Primary Care
    Trust
  • 4 localities North, South, East and West.
  • Service headed by a specialist advisor who has
    operational responsibility.
  • One school health team in each locality providing
    a service for a corporate caseload

  • Safeguarding (child protection)
  • Looked after children
  • Behavioural and emotional problems
  • Children with medical needs/complex needs
  • Immunisations
  • Public health priorities (e.g. mental health)

Supporting Children with medical needs
  • Supporting children with medical needs
  • in schools.
  • Liaison between home/school/health services
  • Education/training on medical conditions
  • Provision of care plans for children with medical
    needs

Supporting children who are on the Code of
Practice.
  • Involvement might be
  • With children who require investigations to
    consider medical/health problems
  • Children with behavioural/emotional difficulties
    which impact on home life as well as school.
  • Liaison between home/school and health services.

Accessing the school health team
  • Each school has a link SH team member
  • This will normally be the person who attends your
    IPM
  • Directly via the team leader
  • Using the referral forms

Referrals
  • All referrals made on a referral form
  • In consultation with and the consent of parents
  • Discussed, prioritised and allocated at weekly
    meeting
  • Allocated to team member with the most
    appropriate skills and knowledge.
  • Individual needs are assessed and care planned
    based on best evidence and practice.
  • Audit
