Provider IVR Quick Reference Guide
  • The following call flow is meant to serve as a
    reference for you to navigate through Elderplans
    new Interactive Voice Response system (IVR).
    When you call the regular Elderplan provider
    service phone number, you will connect with the
    new IVR system. Please have your provider number
    and 5-digit pin available, as well as the
    member id or social security number. The first
    thing you hear will be Thank you for calling
    Elderplan.

  • Press 2 for all provider information. You then
    can follow the rest of the flow below
  • 1-1-1 gt member eligibility current date status
  • 1-1-2 gt member eligibility past dates status
  • 1-2-1 gt members current PCP
  • 1-2-2 gt members PCP for past dates
  • 1-3 gt member co-pay status
  • 2-1 gt search for claim by Elderplan claim number
  • 2-2 gt search for claim by member ID number and
    date of service
  • 3 gt Elderplan provider participation and office
    locations (must have the other providers
    Elderplan ID number)
  • 4-1 gt request Elderplan provider directory
  • 4-2 gt request Elderplan provider manual
  • 4-4 gt request certifax forms
  • 5 gt request authorization or report an emergency
  • 6 gt obtain Elderplan claim mailing address
