Medical Procedures
  • Burger Middle School
  • MEDICATIONS
  • The medication form must be completed for all
    medications and signed by a parent/guardian and a
    physician.
  • This includes over the counter medications, i.e.
    Tylenol. Advil, Tums, Cough Syrup, Dramamine,
    etc This form must be completed in full with
    the following information name of medication,
    dose of medication, frequency or time medication
    is to be given, reason medication is to be given,
    signature of physician and signature of
    parent/guardian. If you would like your child to
    self-administer his/her medication, the physician
    must indicate this on the medication form.
  • All medications must be brought in their original
    containers. All prescription medication must
    have the following information Name of student,
    name of medication, date of prescription,
    expiration date of medication, time medication is
    to be given or frequency of medication, and the
    physicians name. Ask your pharmacist to give
    you another labeled bottle for field trips.
    Please do not send any medications in unmarked
    containers or baggies, they can not be accepted.
  • Please bring enough prescription medication for
    the field trip only. Please bring small
    containers for over-the-counter medications,
    trial sizes are great!

Medical Procedures (cont.)
  • Please give your child his/her morning medication
    before they board the bus, especially Dramamine
    for motion sickness.
  • Please note that the School Nurse Teacher is not
    able to administer any medications, which include
    over-the-counter medications, unless approval
    from a parent/guardian and physician. If your
    child is prone to headaches, motion sickness,
    nausea, diarrhea, or any other illnesses, please
    be sure to plan ahead.
  • Many physicians require two to three weeks to
    complete paper work, please plan ahead and
    consult your physician regarding any medications
    your child will need for the Washington trip.
  • All medication forms must be in the School Health
    Office no later than March 31, 2009, and all
    medications must be delivered by a parent or a
    guardian to the School Health office no later
    than May 1, 2009. The only exception will be new
    medications prescribed from a physician. Your
    child may be excluded from the trip if all forms
    and medications are not in on the due date.
    Please plan ahead.

    Teacher, at (585) 359-5312 with any questions or
    concerns. For your convenience, medication forms
    can be faxed from your physicians office to the
    Burger Middle School Health Office at (585)
    359-5353.

  • 215 pm - Arrive at Gettysburg
  • guided Battlefield Tour
  • 630 pm - Dinner Old Country Buffet

730 pm Depart for Washington to visit the
Jefferson and Washington Memorials 900 pm
Depart for Holiday Inn Laurel MD

Itinerary Thursday May 21st
  • 700 am - Buffet breakfast at hotel
  • 800 am Depart for Washington DC
  • 900 am Group photo on Capitol steps
  • 1000 am Arrive at Capitol Visitors Center for
    guided tour
  • and movie
  • 235 pm IMAX presentation at Air and Space
    Museum
  • 400 pm Dinner at Phillips Seafood Restaurant
  • 600 pm Visit Lafayette Park for White House
    photo op
  • 630 pm visit FDR Memorial, Vietnam Memorial,
    Korean War Memorial, WW II Memorial, and
    Iwo-Jima
  • 1000 pm return to Hotel

    Cemetery Trolley Tour
  • 130 pm Load motor coaches and depart for
    Burger
  • 1130 pm Approximate arrival at Burger

Behavioral Expectations
  • All Burger Middle School rules apply on trip
  • Students may bring electronic devices to listen
    to on bus trip and in hotel rooms.
  • Cell phones may be used on bus trip and in hotel
    rooms only. They are to be left in hotel rooms
    during the day. Cell phones may be used in hotel
    rooms each night. If used during tour they will
    be confiscated and returned on bus trip to
    Burger.
  • Please review in detail the Behavioral
    Expectations form with your child.

Room Sign Up Procedures
  • Room sign up will be in early April
  • Students may choose who they room with
  • Students will be two to a bed
  • We hold the right to change room assignments at
    our discretion

Phone Tree
  • We will put together a phone tree that we will
    activate when we reach Corning NY to let you know
    our approximate arrival at Burger all students
    must be picked up
  • We will also activate phone tree in case of
    emergency
  • Please let me know if you object to being on the
    phone tree and we will not include your name and
    number

Movies/Bags
  • Students are only allowed to bring and view
    movies rated PG
  • Parents must monitor what students bring
  • Parents and students must sign form stating that
    all items in student bag are appropriate (no
    movies rated PG 13 or R, no cigarettes, alcohol,
    tobacco, etc)
  • If a student has to be returned to Burger due to
    safety or insubordination issue Parent must pay
    air travel
