What is an ICP
  • An Integrated Care Pathway is a system which
    facilitates multi-agency care and treatment for a
    specific patient/client group based on guidelines
    and relevant research documents.
  • It should dovetail with existing protocol and
    practice.
  • It forms all or part of the care record,
    documents care given and facilitates the
    evaluation of outcomes for continuous quality
    improvement from a patient and service
    perspective.

What do they do?
  • ICPs involve the whole Multi-Disciplinary Team
    and enables the activities of all the different
    disciplines to be co-ordinated.
  • ICPs are a case management and clinical audit
    tool
  • ICPs define the expected course of events in the
    care of a patient with a specific condition
    within a set timescale.
  • ICPs document what is to be done, by who and when

How do ICPs work?
  • Provide a structure to care which can be audited
    to provide variances from care
  • What are variances?
  • If care is not delivered as planned the reason
    (variance) is completed on the ICP
  • What is variance analysis?
  • A summary of reasons when care is unable to be
    delivered as planned

How do outcomes fit in to pathways
  • Outcome measures allow for trusts to measure the
    effectiveness of the care and treatments it
    provides
  • This will allow for ongoing ICP development
    (using outcomes and variances) to ensure the
    patient receives the best possible care
