  • In our first session, On the road to living well
    with diabetes , we talked about 5 tests, that
    help you know how well your diabetes is doing.
  • The five important diabetes tests are
  • A1c
  • Blood Pressure check
  • Microalbumin test
  • LDL cholesterol test
  • Eye exam
  • In Living Well with Diabetes we will talk more
    about the action steps you can take, to be more
    in charge of your diabetes.

  • At least once a year
  • 2. High blood pressure can cause a lot of
    damage to the body, without us even feeling it.
    The blood pressure check is the only way to know
    if your blood pressure is too high.
  • 3. LDL Cholesterol test lets you know how well
    your heart and arteries are working. Keeping your
    LDL Cholesterol at a healthy level can help lower
    your risk of heart problems.
  • 4. The Microalbumin test lets you know how well
    your kidneys are working.
  • 5. The eye exam shows changes in your eyes that
    can cause vision loss if not treated.
  • Look at your On the Road
  • Booklet, (page 14) from the first
  • class. Have you had a chance to
  • get any of the five tests done?
  • Every 3 4 months
  • 1. The A1c test is considered the most useful
    diabetes test. The A1c number gives an average
    of your blood sugar over the past 3 months.

  • In the On The Road session we talked about some
    of the actions you could take if your test result
    were above the usual goals.
  • The action steps were the same for most all of
    the tests
  • Work out a meal plan that works for you.
  • Be more active.
  • If you weigh too much, try to lose some pounds.
  • If you Smoke, make a plan to stop!
  • Talk to your health care provider about
    medication to best manage your diabetes and
    health.
  • In the Living Well with Diabetes classes we are
    going to talk more about what you can do to plan
    healthy meals and become more active. These are
    two important action steps you can take to feel
    more in charge of your diabetes, while living
    well.
  • We are not able to give individualized diabetes
    management and this is not intended to replace
    diabetes education from a qualified health care
    professional. You will still want to work with
    your health care provider to get all of your
    tests and make sure you are balancing your food,
    activity and medicines to best manage your
    individual needs.
