    selected during annual enrollment, one will be
    assigned to you.

    Annual Earnings (BAE) minimum 20,000 and
    maximum 50,000. Rounded to next 1,000
    multiple.

  • 1, 2, 3, or 4x Basic Annual Earnings (BAE)
    rounded to nearest 1,000. Guarantee issue
    lesser of 2X BAE or 355,000 within first 30 days
    of hire.
  • Evidence of Insurability (EOI) is required when
    applying for greater than 2X BAE or 355,000 or
    after 30 days of hire.

Benefits Summary
  • Employee Assistance Program
  • Allows five (5) face-to-face counseling services.
  • Provides extensive Work/Life Services which
    include resources and referral services for
    childcare, eldercare, household services, pet
    care, concierge services and relocation support.
  • Voluntary ADD
  • Plan offers individual or family coverage.
  • May select any amount of coverage from 25,000 to
    500,000 provided a selection over 250,000 does
    not exceed 10 times your annual salary.
  • Long Term Care (LTC)
  • Purchase LTC coverage for yourself or your family
    members.
  • Choose from three (3) Daily Maximum Benefit (DMB)
    options available - 115 DMB, 230 DMB and 345
    DMB.
  • Flexible Spending Account (FSA)
  • Health Care FSA - Minimum contribution of
    300/year to maximum contribution of
    5,000/year. (Pomona College Maximum
    contribution of 8,000/year.)
  • Dependent Care FSA - Minimum contribution of
    300/year to maximum contribution of
    5,000/year.
  • If you do not want to make FSA election changes,
    you do not need to make changes on Web.
  • (EXCEPT Pomona Colleges requires FSA elections
    each year.)

(FSA) Summary Integration of FSA with Health
Savings Account (HSA)
  • FSAs can be offered alongside an HSA only if the
    FSA provides limited scope benefits.
  • Limited-scope FSA only reimburses expenses not
    reimbursed under HSA such as dental and vision.
    Eligible medical expenses will be reimbursed by
    the FSA only after the HDHP deductible HSA been
    satisfied.
  • If only the employee is enrolled in a HDHP, FSA
    funds (except limited scope dental/vision) are
    not available for any family member until
    employee HSA satisfied the deductible.
  • EXAMPLE Employee enrolls in HDHP but spouse
    and children are enrolled in spouses Kaiser
    Plan. Medical expenses for dependents will not
    be reimbursed by FSA until employee satisfies
    deductible.
  • Dependent Care FSA is not affected and will
    continue to be offered.
  • If you do not elect an HSA you may participate in
    the full scope FSA. Full scope FSA includes
    reimbursement for all medical expenses as defined
    by IRS regulations.

(HSA) Summary Enrollment / Contribution Process
  • Enrollment
  • Employee enrolls in HDHP via web. Employee can
    elect to enroll in HSA. HSA enrollment is not
    mandatory. May elect to enroll in Mellon HSA or
    other.
  • If employee elects to enroll into Mellon HSA this
    election is sent to Anthem Blue Cross. Within
    two weeks Lumenos will send a Welcome Kit to
    employee home address. Inside welcome kit is
    signature card. Signature card needed to order
    checks and beneficiary designation. Note Do not
    have to order checks, but should elect
    beneficiary. Employee sends enrollment materials
    directly to Mellon HSA. Account is opened within
    48 hours of receiving signature card.
  • Mellon HSA Debit cards take two weeks upon
    receipt of enrollment materials. Employee may
    contact customer services to request additional
    debit cards for dependents. Additional card will
    be in dependents name and send to employee home
    address.
  • Employee may elect to enroll HSA contributions to
    other bank HSA. (The Claremont Colleges will
    not be responsible for providing enrollment
    materials or information regarding other HSA
    financial institutions.)
  • HDHP/HSA I.D. cards will be mailed the first of
    the year. Once you receive an I.D. card you may
    also enroll in the Mellon HSA via web. Accounts
    are opened immediately if enrolled via web with
    e-signature.
  • The member can withdraw funds using either the
    debit card or check.
  • Contributions
  • Pre-tax contributions through payroll deduction
    will be sent to Mellon beginning 1st payroll in
    January.
  • Employees can deposit money on an after-tax basis
    (via check or electronic transfer) directly into
    their Mellon HSA account.
  • Investment Options
  • With certain amount of funds in HSA, you will
    have various investment fund options.
