Welcome to Fort Gordon
  • As a new employee, we welcome you to the Army
    Civilian Personnel Community. You will report to
    Darling Hall, Bldg 33720, Room 229, 307
    Chamberlain Avenue, 0800 on Enter On Duty Date
    (EOD), for in-processing.
  • On the next slide you will be given specific
    information on your benefits. Please read this
    information first. It contains the deadlines to
    sign-up for your benefits.
  • Next, you will find a list of the forms well
    need you to fill out and special instructions to
    help you in that process.
  • This office must receive the completed, original
    forms as stated in the e-mail/letter. If the
    forms are not received on time, your report date
    will be changed to a later date. Our mailing
    address is listed on the last slide.

    information carefully. The responsibility for
    signing up for benefits through the Army Benefit
    Center within the allotted time frames, is solely
    the responsibility of the employee.
  • As a new hire you will have to wait approx. a
    week after in-processing to elect your benefits.
  • At the end of this presentation you will also
    find information on the new Federal Employees
    Dental and Vision Insurance Program (FEDVIP),
    which are supplemental dental and vision
    packages.

  • SF 144 - Statement of Prior Federal Service
  • Fill out blocks 1-3, question 4 asks if the
    information contained on your resume reflects all
    your Federal government civilian and uniformed
    service, if your answer is yes, skip to question
    8, and then sign and date.
  • If your resume does not reflect all your service,
    please use Section 5 to show the service.

    on the day of your in-processing

Special Instructions for the following forms
  • Form I-9 Employment Eligibility Verification
  • To ensure citizenship and Employment Eligibility,
    please provide the following on the day of your
    in-processing
  • If you have a passport (an expired passport is
    valid) this will be the only document required
    for this form.
  • If you dont have a passport
  • Please bring one of the items listed on the
    second page List of Acceptable Documents
    from List B And
  • One of the items listed on the second page -

Special Instructions for the following forms
  • SF1199A - Direct Deposit Sign-Up Form
  • To sign you up for Direct Deposit we will need
    for you to either
  • 1) Take this form to your bank or credit
    union. Section 3 should be filled out and
    signed by them.
  • Or
  • 2) Provide a voided check at the time of your
    in-processing. Please do not sign the check, but
    ensure that the word void is clearly written
    across the check.

Army Pay Days
  • Your first pay day with Army will be Thursday, 2
    weeks after your EOD. Pay periods begin on a
    Sunday, and end two weeks later on a Saturday.
  • You will see that your pay days will follow every
    two weeks on Thursday.
  • Your Leave Earnings Statement (LES) will be
    available to you at https//mypay.dfas.mil/mypay.a
    spx
  • Pay problems should be reported immediately to
    your CPAC Representative Customer Service Rep
    (CSR) 791-1371.

Special Instructions for the following forms
  • SF1152 Designation of Beneficiary (Unpaid
    Compensation of Deceased Civilian Employee)
  • This beneficiary form is used to pay the
    remaining salary and annual leave, and any other
    payments (i.e., unpaid travel, etc.) that are due
    at the time of your death.
  • Please do not erase or cross out on the form, use
    a new one, and make sure you sign at the time of
    your in-processing in front of witnesses.

Special Instructions for the following forms
  • SF2823 Designation of Beneficiary Form for
    Federal Employees Group Life Insurance (FEGLI)
  • If you plan to keep basic life insurance or make
    additional elections, this form is used to pay
    your beneficiaries. Please ensure that you have
    social security numbers listed.
  • Please do not erase or cross out on the form, use
    a new one, and make sure you sign at the time of
    your in-processing in front of witnesses.

  • SF3102 Designation of Beneficiary Form for
    Federal Employees Retirement System (FERS)
  • This form is used to pay your beneficiaries the
    balance in your retirement system.
  • Please do not erase or cross out on the form, use
    a new one, and make sure you sign at the time of
    your in-processing in front of witnesses.

Federal Employees Dental and Vision Insurance
Program (FEDVIP)
  • Introduction
  • The Federal Employee Dental and Vision Benefits
    Enhancement Act of 2004 requires OPM to establish
    arrangements under which supplemental dental and
    vision benefits will be made available to federal
    employees, retirees, and their dependents.
  • The new Federal Employees Dental and Vision
    Insurance Program (FEDVIP) will be available to
    eligible Federal and Postal employees, retirees,
    and their eligible family members on an
    enrollee-pay-all basis. This new Program allows
    dental and vision insurance to be purchased on a
    group basis which means competitive premiums and
    no pre-existing condition limitations. Premiums
    for enrolled Federal and Postal employees will be
    withheld from salary on a pre-tax basis.
  • Eligible individuals can enroll in a dental plan
    and/or a vision plan. They may enroll in a plan
    for Self-only, Self plus one, or Self and family
    coverage. Eligible family members include an
    enrollee's spouse and unmarried dependent
    children under the age of 22, or if age 22 or
    older, incapable of self-support. The rules for
    family members' eligibility are the same as they
    are for the FEHB Program.
  • Employees must be eligible for the FEHB Program
    in order to be eligible to enroll in FEDVIP. It
    does not matter if they are actually enrolled in
    FEHB - eligibility is the key. Annuitants are
    eligible to enroll in FEDVIP no matter what their
    FEHB status is.
