2Going to a Provider
- Provide your new insurance information when you
call to make the appointment. - No payment at time of service. Your provider
will code the visit and bill it to insurance
(reminder to providers on the back of your Cigna
ID card). - Cigna will send you an Explanation of Benefits
(EOB) advising the negotiated rate. - Preventive care paid at 100 in-network all
other services subject to the deductible and
co-insurance.

You have Automatic Claims Forwarding (ACF), so
Cigna withdraws funds from HSA to pay provider

4Prescription Benefits
- Prescriptions are the only item you should pay
for at time of service. - You will be paying the negotiated rate there
are no co-pays with this plan. - Co-insurance amounts take effect once you have
met your deductible. - If you pay out of pocket, you can reimburse
yourself for the cost of the prescription with
your HSA check book or debit card. - It is important to keep your receipts.

6Your Health Savings Account
- Diocese contributes 80 of deductible for 2008.
- Initial funding equals one quarter of the
diocesan contribution and the remaining funding
is paid out monthly starting in February. - Employee has the option to fund the remaining IRS
limit ( 5800 Family, 2900 Single) into the HSA
through payroll deduction or a personal or Church
check made out to the Diocese of Texas.
7Tax Information
- JP Morgan Chase will provide you and the IRS the
following annually - Form 5498-SA reports HSA contributions made by
your employer and you - Form 1099-SA reports all HSA distributions
- You will be responsible for completing Form 8889
along with your Form 1040 - KEEP YOUR RECEIPTS!
