- Premium monthly amount to have coverage
deducted pre-tax from payroll - Co-payment fixed amount you pay for a specific
activity (i.e., doctors appt.) - Deductible amount you pay first, before
insurance pays - Co-insurance percentage you pay after paying
your deductible
6Eligible Dependents
- Who may be covered?
- Full-time faculty or classified employee
- Legally married spouse of employee
- Unmarried dependent children until the end of the
year in which they turn age 23 (receiving over
one-half support from employee living at home
or away at school) - Unmarried biological or adopted children
- Unmarried stepchildren living full time with
employee in parent-child relationship and
dependent based on IRS standards - Other children, if in court-ordered custody of
the employee
7Disabled Dependents
- At the end of the year in which your dependent
child turns 23, he/she will be dropped from your
health insurance - We will send a notice to you and your child in
the fall - If your child is disabled
- You must work with Anthem before December 31 of
that year to certify the disability and be
approved for continuing coverage - Anthem will require periodic review of disability
status
8Flexible Reimbursement Accounts (FRAs)
Use pre-tax dollars to be reimbursed for eligible
expenses

- - Child or adult care
- - 5,000 maximum married filing jointly or
single w/children - - 2,500 maximum married filing separately

- - Must be medically necessary and IRS eligible

10Life Events
- Outside of Open Enrollment, only way to make
changes is through qualifying mid-year (life)
event - Can change plan, add/drop expanded coverage with
life event - Must make changes within 31 days of event
- Effective 1st of month following receipt of form
or E-direct change - Exception - newborns added effective 1st of month
of birth ineligible dependents dropped 1st of
month after event
