Health Care Terminology
  • Premium monthly amount to have coverage
    deducted pre-tax from payroll
  • Co-payment fixed amount you pay for a specific
    activity (i.e., doctors appt.)
  • Deductible amount you pay first, before
    insurance pays
  • Co-insurance percentage you pay after paying
    your deductible

Eligible Dependents
  • Who may be covered?
  • Full-time faculty or classified employee
  • Legally married spouse of employee
  • Unmarried dependent children until the end of the
    year in which they turn age 23 (receiving over
    one-half support from employee living at home
    or away at school)
  • Unmarried biological or adopted children
  • Unmarried stepchildren living full time with
    employee in parent-child relationship and
    dependent based on IRS standards
  • Other children, if in court-ordered custody of
    the employee

Disabled Dependents
  • At the end of the year in which your dependent
    child turns 23, he/she will be dropped from your
    health insurance
  • We will send a notice to you and your child in
    the fall
  • If your child is disabled
  • You must work with Anthem before December 31 of
    that year to certify the disability and be
    approved for continuing coverage
  • Anthem will require periodic review of disability
    status

Flexible Reimbursement Accounts (FRAs)
Use pre-tax dollars to be reimbursed for eligible
expenses
  • Dependent Care Expenses
  • - Child or adult care
  • - 5,000 maximum married filing jointly or
    single w/children
  • - 2,500 maximum married filing separately
  • Medical Care Expenses
  • - Must be medically necessary and IRS eligible
  • - 5,000 limit

  • Can be reimbursed for certain over-the-counter
    expenses, including
  • Allergy remedies
  • Antacids
  • Cold remedies
  • Pain remedies
  • Eye products
  • Contact lens cleaning solution
  • Reading glasses
  • Nicotine gum or patches for smoking-cessation
    purposes

Life Events
  • Outside of Open Enrollment, only way to make
    changes is through qualifying mid-year (life)
    event
  • Can change plan, add/drop expanded coverage with
    life event
  • Must make changes within 31 days of event
  • Effective 1st of month following receipt of form
    or E-direct change
  • Exception - newborns added effective 1st of month
    of birth ineligible dependents dropped 1st of
    month after event
